Epidemiology and Etiology of lipomas

Lipoma is a soft, fatty lump beneath the skin that occurs due to an overgrowth of fat cells. Doctors consider lipomas to be benign tumors, meaning it is not cancerous. There is a rare form of cancer known as liposarcoma that occurs within fatty tissue and may look like a deep lipoma. Lipomas can also be associated with syndromes such as adiposis dolorosa, hereditary multiple lipomatosis, Madelung’s disease and Gardner’s syndrome. The etiology of lipomas still remains indecisive and there are several theories on store. One hypothesis suggests that trauma fuels cytokine release, triggering pre-adipocyte differentiation. Another posits that trauma causes fat herniation, leading to the formation of pseudo-lipomas. However, no definitive link has been established between trauma and lipoma formation.

Lipomas can crop up anywhere on the body where fat cells are present, but they tend to appear on the trunk, chest, neck, shoulders, armpits and thighs. In less common cases, lipomas can develop deeper inside the body in internal organs, bones, or muscles. They are ‘doughy’ to touch and feels like it can move. Lipomas range from the size of a pea to a few centimeters across. Sometimes larger lipomas do occur, with some reaching almost 10 centimeters across.

When is lipoma removal necessary?

• Increasing in size or rapid growth
• Causing visible changes in the overlying skin and becoming red or hot
• Turning into a resolute or immovable lump
• Causing distress for cosmetic reasons
• Causing bothersome symptoms and interferes with normal body functions.

Lipoma removal treatments

Lipoma treatments include:

• Surgical removal – Most lipomas are removed surgically which is the most recommended procedure and is done by cutting them out. Relapse after removal is uncommon. Possible side effects are bruising and scarring.
• Liposuction: Typically performed as a cosmetic procedure, liposuction is used to remove fat. Liposuction leaves less scarring, though, there are chances for re-occurrence of lipomas. There are different liposuction techniques exist to facilitate the proper removal of adipose tissue.
• Lipolytic injections or Injection lipolysis: Injection lipolysis is a rapidly growing technique for non-surgical body contouring and dissolving localized collections of excess fat. Here the lipodissolve  solutions (like phosphatidyl choline/ sodium deoxycholate (PDC/DC) compounds) are injected to destroy fat cells that cause lipomas. PDC/DC has yielded a great decrease in lipoma size in multiple studies when applied to lipomatous tissues.
